We are happy to inform you that your child’s school has an on-site School-Based Health Center (SBHC)!
The SBHC is staffed by licensed health professionals from Urban Health Plan.
Urban Health Plan (UHP) is a network of community health centers in the Bronx, Queens and Harlem. We
have provided quality, affordable primary and specialty care in the Bronx community for over 45 years.
At the SBHC, your child can get medical services at no cost to you, regardless of insurance status.
